The following form needs to be completed by each bowler at check-in before entering the bowling center for our events. This form needs to be completed to state the bowlers names and guests – each bowler is limited to a max of two guests following all PJBT Covid-19 Protocols.
All people entering our events currently must complete this form and be temperature checked before entering.
